Process for Obtaining an OHF AAA Hockey Waiver
Here is the process if a player is released (cut) and wishes to tryout in another centre. ...
1. The coach will communicate that the player will not be continuing with the tryout. This is done in a variety of ways, such as email, a brief conversation, or a letter.
2. If the player wishes to tryout in a different centre, they need to...
a) Obtain an OHF AAA Hockey Waiver, AND
b) Their signed-off permission to skate form which was submitted at the beginning of the tryout process.
The OHF Waiver is a Carbon Copy Form issued by the Ontario Hockey Federation and OMHA. It must be signed in person by a representative of QRD (usually Lesley Turcotte and Greg Popovich), and the parent and player. We usually have people prepared to sign waivers at our home rinks during the tryout process. If this is not possible you can make special arrangments with Lesley at 613-921-5161.
Once the waiver is signed, you can make arrangements to have Lesley email the signed PTS form back to you.
