Please read this before proceeding. This form is to be used to schedule exhibition games, and to request a sanction/travel permit for exhibition games. If your team is playing exhibition games AT another centre, in Section III you only need to list the QRD games. If you are hosting multiple exhibition games, ALL match-ups need to be shown so that they can be scheduled. If it is a single exhibition game, please indicate the opponent.
