General Instructions: 1. Please fill out this form completely. 2. An email address is required to complete the form. It will be used for all further communication. Please ensure that it is typed in correctly. 3. The U10 & U11 group will have 4 Prep Skates. The group is limited to 20 skaters and 4 goalies. Goalies must register on separate form. NOTE: Players must have been rostered to Quinte or feeder associations (rostered in the 2025/2026 season) ie at U9MD or U9 Select and Below are eligible to attend prep skates. Players currently not rostered or insured with sanctioned programming (ie registered with KLEVR or ORHL) are not eligible.
