NOTE: Players must have been rostered to Quinte or feeder associations (rostered in the 2025/2026 season) to participate.

General Instructions:
1. Please fill out this form completely.

3. The U16 & U18 group will have 4 Prep Skates. Space is limited to 4 goalies. Skaters must register on the skater form.

4. The cost for goalies is $80/goalie for the 4 sessions.
